RENTAL AGREEMENT: The customer is responsible for:
1. Protecting the equipment from theft, fire, water or other damage while in the customer's care.
2. Using the equipment for which it was prescribed and for whom it was intended.
3. Notifying Affinity Medical immediately of any equipment failure, damage, change of personal info, or payment information.
4. Agrees to pay to fix or replace any item damaged due to customer abuse (not normal wear) during rental period.
HOLD HARMLESS: Affinity Medical is not responsible for any damage or injury to any person or property from the use of equipment or services from Affinity Medical and agrees that the customer's sole remedy for any such damages will be to bring such action solely against the manufacturer of such equipment.
CANCELLATIONS:
As a courtesy to other customers requiring the use of our equipment, Reservations may be cancelled at any time up to 7 days prior to the delivery or pick date at no cost.
Cancellations less than 7 days prior will have a cancellation fee of 50% of the total rental contract price, should the rental contract be cancelled on the day of delivery or after delivery of the equipment we will charge 100% of the total rental contract price.
ACCEPTANCE: I accept and understand the above terms & condition's. I further acknowledge financial responsibility for charges related to these transactions
