I, for myself and any minor children for which I am a parent, legal guardian or otherwise responsible, release the bicycle shop, agents and employees from any liability for damage or injury to myself or any person or property resulting from negligence, adjustment, selection and use of this equipment. I, understanding the inherent risk involved in using this equipment, accept the full responsibility for any and all such damage or injury which may result.  By signing this form I agree that I have received instruction on how to use the rental equipment, that helmets must be worn while riding bikes at all times, have been offered a helmet for use, and have been given ample opportunity to ask questions regarding the operation of rental equipment.

I accept for use, as is, the equipment in good condition and accept full responsibility for care of equipment while in my possession. I will be responsible for the prompt replacement at full retail value of all rental equipment not returned or damaged, other than reasonable wear and tear, which results from the use of this equipment.

I agree to return rental equipment by agreed upon date in clean and undamaged condition to avoid additional charges.  Staff will determine if equipment is returned clean, and assess damages upon return.  It is the customer's responsibility to note existing damages to equipment at time of rental pick-up.

All instructions on the use of rental equipment have been made clear to me and I understand the function of the equipment.

I accept the terms of this agreement and accept responsibility for the above charges.

I understand that a rental must be returned before store closing each day, with exception of multi-day rentals where the bikes must be returned prior to closing the last day of the rental. Failure to return the bike(s) by the designated time will be charged accordingly.

If signing as Parent/Guardian/Agent: I verify that I am the parent, guardian or agent of the Equipment User and that I have the authority to enter into this agreement on behalf of the Equipment User and I agree to be bound by the terms of the Warning, Assumption of Risk, Liability release, Indemnity and Hold Harmless Agreement and Agreement Not to Sue.
