I hereby certify that I have this date received the above equipment and acknowledge that I personally inspected it and that it was delivered to me in good working condition.  I agree that i am solely responsible for any damage to or loss of said equipment and agree to return the equipment in the same condition in which I received it.  I understand that I will be charged for any damage caused to the equipment during the duration for the agreement.  Bike helmet required.

I understand that bicycling is an inherently dangerous activity that can result in serious injury or death.  I am familiar with the use and operation  of said equipment and I agree to use the equipment solely for the purpose for which it is designed.  I acknowledge that I am assuming all risk  caused by my use of said equipment and the Cordova Gear has no duty supervise my conduct.

It is expressly understood and agreed that Cordova Gear shall not be liable for any damages whatsoever arising from injuries to persons and /or damage to property arising from the use , transporting or operation of, or in any way connected with said equipment or any part thereof, I further agree to indemnify Cordova Gear and hold it harmless from any and all such liability.

I understand the cancellation policy: if reservation is cancelled prior to one month of the reservation beginning date 50% of rental fee will be refunded. Cancellation within 30 days of reservation start date will result in no refund.
