Bike Rental Injury and Damage Liability Disclosure

I, myself (the renter) and any minor children for whom I am parent, legal guardian or otherwise responsible, release the Mountain Life Supply co, LLC, it’s owners, agents, and employees from any liability for damage or injury to myself or any person or property resulting from negligence, adjustment, selection and use of this equipment. I, understand the inherent risk involved in using this equipment, accept the full responsibility for any and all such damage or injury, which may result.

I accept for use, as is, the equipment in good condition and accept the full responsibility for care of equipment while in my possession. I will be responsible for the prompt replacement at full retail value of all rental equipment damaged or not returned to Mountain Life Supply co. Damage, other than reasonable wear and tear, which results from the use of this equipment; I will take responsibility to communication with Mountain Life Supply co, LLC if any issues, damage or stolen equipment. I agree to return rental equipment by agreed time/date in the same condition as when it was received to avoid additional charges. I, the renter, will accept responsibility for lost or stolen equipment up to full retail cost of the rented equipment.

Late Fees: A late fee may be charged to I, the renter, by Mountain Life Supply co the owner at a rate of $15 per 30 minutes late from agreed upon return time. If the bike is 2 hours late or more, Mountain Life Supply co reserves the right to charge the renter for one more day of rental fees < $120.

If I, the renter fails to communicate to Mountain Life Supply co, concerning tardiness we will assume the bike is stolen and the customer will be charged the full retail price of that bike and/ up to being charged with stolen property with the police department.

All instruction on the use of the equipment has been made clear to me and I understand the function of the equipment. I take full responsibility for any injury incurred by mountain biking and will not pursue action against Mountain Life Supply co, LLC.

I, the renter or guardian of minors, will take full responsibility for the care and return of the mountain bikes owned and serviced by Mountain Life Supply co, LLC. If renting an e- Bike, I the renter must return all parts for charging the battery back to Mountain Life Supply co, LLC
